29 CFR 1910.109(b)(1): Explosives or blasting agents were stored in a manner that such storage of explosives or blasting agents constituted an undue hazard to life.    a) On or about August 31, 2011, in the flammable storage room of the Idaho City District:  Boxes of Orion Highway Safety Flares were vertically stacked, and adjacent to flammable and combustible liquids in the Flammable Storage Building.  The safety flares are classified as Class 1.4S Flammable Solids on its corresponding MSDS.  The MSDS states that materials to avoid include organic solvents and gasoline.    NOTE: Abatement certification AND supporting documentation are both required for this item.

29 CFR 1910.303(g)(1)(i)(B): The width of working space in front of electric equipment was not the width of the equipment or 762 mm (30 in.), whichever is greater.  The working space did not permit at least a 90-degree opening of equipment doors or hinged panels.    a) On or about August 31, 2011;  Fire Engine Bay - The width of the working space in front of the electric panel was blocked by a television and mounting table, and there was not at least 30 inches of working space to permit at least a 90-degree opening of the hinged panel.
