29 CFR 1926.503(a)(1):     The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    Salgau Roofing, Inc., conducting work at 10621 Fremont Pike, Perrysburg, OH: On or about September 12, 2011, the employer failed to ensure that employees had been trained in the appropriate use of fall protection such as, but not limited to, the use of personal fall arrest systems, or alternative measures of fall protection while replacing 5/12 pitch roof of the two story hotel building. Employees did not use fall protection and were exposed to approximately 18 feet fall hazard from the roof to the ground below.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1926.1060(a):     The employer did not provide a training program for each employee using ladders and stairways, that enabled each employee to recognize hazards related to ladders and stairways, and trained each employee in the procedures to be followed to minimize those hazards:    Salgau Roofing, Inc., conducting work at 10621 Fremont Pike, Perrysburg, OH: On or about September 12, 2011, the employer did not ensure that employees, using an extension ladder to access the roof of the two story hotel building in order to conduct roofing activities, were trained to recognize the hazards associated with the ladders use and to understand the procedures to control or minimize those hazards. Employees did not extend the ladder at least 3 feet above the roof edge and were exposed to approximately 18 feet fall hazards from the ladder to the ground below.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. Employees were exposed to approximately 18 feet fall hazard from the roof to the ground below.    No additional abatement information is needed for this item.

29 CFR 1926.1053(b)(1):     Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    Salgau Roofing, Inc., conducting work at 10621 Fremont Pike, Perrysburg, OH: On or about September 12, 2011, the employer did not ensure that an extension ladder, used to access the roof of the two story hotel building, was extended at least 3 feet above the roof edge. The employees were exposed to approximately 18 feet fall hazards when using the ladder to access the working level in order to conduct roofing activities.    Salgau Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1923.1053(b)(1)), which was contained in OSHA Inspection Number 309449825, citation number 1,  item number 6, and was affirmed as a final order on November 28, 2007, with respect to a workplace located at 2921 Collingwood Blvd., Toledo, OH.    No additional abatement information is needed for this item.
