29 CFR 1926.503(a)(1):     The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    Salgau Roofing, Inc., conducting work at 10621 Fremont Pike, Perrysburg, OH: On or about September 12, 2011, the employer failed to ensure that employees had been trained in the appropriate use of fall protection such as, but not limited to, the use of personal fall arrest systems, or alternative measures of fall protection while replacing 5/12 pitch roof of the two story hotel building. Employees did not use fall protection and were exposed to approximately 18 feet fall hazard from the roof to the ground below.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

Employees did not extend the ladder at least 3 feet above the roof edge and were exposed to approximately 18 feet fall hazards from the ladder to the ground below.
