adm'd to 1926.502(b)(1) Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels is not protected by guardrail systems, safety net system, or personal fall arrest system nor are employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): a) Salgau Roofing, Inc. on or about July 30, 2021: The employer does not require the use of any form of fall protection during roofing work. The violation was most recently observed at 2829 Westchester Rd Ottawa Hills, Ohio, where employees were not protected from a 25 ft. fall to the ground below. To abate this hazard the employer must ensure that its employees performing residential roofing work are protected from falls, and no employees are allowed onto a roof more than 6 feet high without adequate fall protection. Salgau Roofing Inc. was previously cited for a violation of an equivalent Occupational Safety and Health standard 1926.501(b)(13) which was contained in OSHA inspection number 1367041 citation number 1, item number 1 and was affirmed as a final order on March 13, 2019 with respect to a workplace located at 632 Manitou Dr. Maumee, OH Salgau Roofing Inc. was previously cited for a violation of an equivalent Occupational Safety and Health standard 1926.501(b)(13) which was contained in OSHA inspection number 1265490 citation number 1, item number 1 and was affirmed as a final order on August 1 ,2018 with respect to a workplace located at 6550 Cordurory Road Oregon, OH

29 CFR 1926.453(b)(2)(v):A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: a) Salgau Roofing, Inc. on or about July 30, 2021: The employer did not ensure that a worker operating an aerial lift basket connected a lanyard to the basket while conducting roofing activities from a height greater than 25' while accessing a residential structure roof, thereby exposing employees to a fall hazard of approximately 25 feet.
