29 CFR 1926.502(f)(1)(i):When no mechanical equipment was being used the warning line was not erected at least 6 feet (1.8m) from the roof edge: Salgau Roofing, Inc. - On or about December 9, 2021, at the worksite located at the NW corner of N. Summit and Locust Streets in Toledo, Ohio: The employer did not adequately protect employees from fall hazards in that the warning line system was not erected at least six feet from the unprotected edges of the roof. Employees were exposed to an approximate ten foot fall hazard. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.502(h)(1)(v):The safety monitor had other responsibilities which could take the monitor's attention from the monitoring function: Salgau Roofing, Inc. - On or about December 9, 2021, at the worksite located at the NW corner of N. Summit and Locust Streets in Toledo, Ohio: The employer did not adequately protect employees from fall hazards in that a safety monitor performed roofing tasks. Employees were exposed to an approximate ten foot, and up to 15-foot fall hazard while the safety monitor was not performing monitoring functions. No additional abatement information is needed for this item.
