29 CFR 1910.213(b)(3): On applications where injury to the operator might result if motors were to restart after powerfailures, provision shall be made to prevent machines from automatically restarting upon restoration of power. Logistic Warehouse wood shop: On or about September 7, 2011 and times prior thereto, employees were using a skill saw tablesaw that would automaticaly restart once power was restored. This exposed employees to possible amputation to hands or struck-by hazards from wood being thrown from the saw if it was to restart.

29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear. Logistic Managers Office: On or about September 7, 2011 and times prior thereto, an 120 volt 15 amp receptacle was missing the cover. This exposed employees to electric shock to include electocution if they contacted the conductors.
