29 CFR 1910.36(g)(2): An exit access must be at least 28 inches (71.1 cm) wide at all points. Where there is only one exit access leading to an exit or exit discharge, the width of the exit and exit discharge must be at least equal to the width of the exit access. (a) Logistics Warehouse Building 170: On or about 8 September 2011, and times prior thereto, the access through an walkway in the Northeast side of the warehouse was limited down to 18 inches by shelving and other materials; limiting egress in the event of a fire.

29 CFR 1910.37(b)(4): If the direction of travel to the exit or exit discharge is not immediately apparent, signs must be posted along the exit access indicating the direction of travel to the nearest exit and exit discharge. Additionally, the line-of-sight to an exit sign must clearly be visible at all times. (a) Logistics Warehouse Building 170: On or about 8 September 2011, and times prior thereto, directional signage to the exit locations were not present in the open warehouse area.

Employers must install and maintain an operable employee alarm system that has a distinctive signal to warn employees of fire or other emergencies, unless employees can promptly see or smell a fire or other hazard in time to provide adequate warning to them. (a) Logistics Warehous Building 170: On or about 8 September 2011, a fire alarm was installed above the ceiling tiles of facility in the administrative customer reception room in the dock area of the warehouse; exposing employees to fire and smoke inhalation hazards in the event of a mishap.
