29 CFR 1926.1437(h)(6): The employer did not ensure that the information required in 1926.1437(h)(2) for monthly barge crane inspections was documented and maintained in accordance with 1926.1412(e)(3): (a) The inspection revealed that the employer did not document the items checked, the results of the inspection, the name and signature of the person who conducted the inspection and the date for monthly barge crane inspections of the "Betsy Ross". The barge was in use on the Columbia River at Bonneville Dam at the time of the OSHA inspection.
