29 CFR 1926.303(c)(3): All other portable abrasive wheels used for external grinding, were not provided with safety guards (protection hoods) meeting the requirements of paragraph (c)(5) of this section: (a) In Building A's public toilet area on or about November 7, 2011, there was no guard for the 4 inch diameter mason's abrasive wheel on a 110 VAC Makita rught angle grinder. The grinder was used to cut into the concrete floor to install plumbing. The smooth edge of the cut was observed about one deep into the concrete

29 CFR 1926.404(b)(1)(i): The employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites: (a) In Building A's public toilet area on or about November 7, 2011, there was no assured equipment grounding conductor program or ground-fault circuit-interrupter protection when an extension cord was used to power a 110 VAC Makita right angle grinder and battery chargers. THE JUNG IN COMPANY WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.404(B)(1)(I) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 312495997, CITATION NUMBER 1, ITEM NUMBER 1, ISSUED ON FEBRUARY 2, 2009, WITH RESPECT TO A WORKPLACE LOCATED AT 91 LOT 3 R2, EAST LIGUAN AVENUE, DEDEDO, GUAM.
