29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: At the back wall of the building, the ground pin was missing on an extension cord that supplied 110 VAC power to a Bosch 11255VSR Bulldog Extreme, 1 rotary hammer. Employee chipping concrete using the rotary hammer. Employee was exposed to the hazard of contacting electricity.
