29 CFR 1910.22(a)(1):  All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition:    a)  A & D Wood Products, Inc., Elida, Ohio:  On or about November 8, 2011, the employer did not ensure that work areas, such as, but not limited to those around the Baker Product Baker Band Resaw were maintained in a clean and orderly condition.  Saw dust and wood debris had been allowed to accumulate in working area, exposing employees to slip and trip hazards.

Employees were exposed to eye injuries from flying particles.    b)  A & D Wood Products, Inc., Elida, Ohio:  On or about November 8, 2011, the employer did not ensure that each affected employee used appropriate eye protection while engaged in cutting activities on equipment, such as, but not limited to the H&A Saws, Inc. manufactured swing cutoff saw (Model 55, Serial #8519).  Employees were exposed to eye injuries from flying particles.

29 CFR 1910.147(c)(1):  The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a)  A & D Wood Products, Inc., Elida, Ohio:  On or about November 8, 2011, the employer failed to establish energy control procedures to protect employees from amputation hazards on equipment while exposed to unexpected startup and energy releases before directing employees to perform maintenance and servicing work on equipment, such as, but not limited to the Smart Products Pallet Saw and the Baker Products Baker Band Resaw.  Employees changed the blade on the saw without locking it out.

29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings were not protected from abrasions:    a)  A & D Wood Products, Inc., Elida, Ohio:  On or about November 8, 2011, the employer did not ensure that conductors entering the cabinet mounted on the H&A Saws, Inc. manufactured swing cutoff saw (Model 55, Serial #8519) were protected from abrasion and that the opening through which the conductors entered the cabinet was effectively closed.  The swing cutoff saw was located in building #2 and exposed employees to electrical hazards.
