29 CFR 1910.95(g)(6):     At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels :    (a)  The employer did not continue to provide annual audiograms as part of an effective hearing conservation program for employees exposed to to an 8-hour time weighted average noise above 85 dBA as part of an effective hearing conservation program.  Employees are at increased risk of not being notified of potential hearing loss identified by standard threshold shifts of hearing function.

29 CFR 1910.334(a)(2)(ii):     There was a defect or evidence of damage that could have exposes an employee to injury and the defective or damaged item was not removed from service:    (a)  A portable blue Patterson flan near the wheelabrator had a dammaged electrical cord.  The plug's strain releif was disattached from the cord's  sheathing and the plug's grounding pin was missing.  Employees were exposed to electrical shock and fire hazards.     (b)  A black Dayton pedestal fan had a damaged electrical cord.   The plug's housing was damaged and the cord's strain relief was missing.  Employees were exposed to the inner current carrying wires and possible shock.

29 CFR 1910.1000(a)(2):     Employee(s) were exposed to an airborne concentration of copper fume listed in Table Z-1 in excess of the 8 hour Time Weighted Average concentration of 0.1 milligrams of copper fume per cubic meter of air:    (a)  An air arc welder was exposed to copper fume at an 8-hour time weighted average of 0.25 mg/m3, approximately 2.5 times the limit of 0.1 mg/m3.  This limit is established to prevent upper respiratory tract irritation and metal fume fever.  The samples were collected on December 1, 2011 during a 442 minute sampling period.  Exposure calculations include a zero increment for the 38 minutes not sampled.

Improving local exhaust ventilation at the work station;  2. Employee rotation.
