29 CFR 1910.134(g)(1)(i)(A): Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function: (a) On or about November 18, 2019, an employee preparing castings, used a 3M half face elastomeric respirator while blasting and doing other tasks such as, welding, sanding and grinding. The employee was not clean shaven affecting the tight fitting seal of the respirator. This exposed the employee to inhalation of airborne metal dust generated during these operations. (b) On or about 1/28/2020, an employee preparing castings, used a 3M half face elastomeric respirator while blasting and doing other tasks such as, welding, sanding and grinding. The employee was not clean shaven affecting the tight fitting seal of the respirator. This exposed the employee to inhalation of airborne metal dust generated during these operations.

29 CFR 1910.134(k)(1)(i): The employer did not ensure that each employee could demonstrate knowledge of why the respirator was necessary and how improper fit, usage, or maintenance could compromise the protective effect of the respirator: (a) On or about November 18, 2019, an employee preparing castings and using a 3M half face elastomeric respirator while blasting and doing other tasks such as, welding, sanding and grinding, was not clean shaven. The employee was not trained on this type of respirator and the proper use of the respirator.
