29 CFR 1910.23(b)(2):     Every chute wall opening from which there is a drop of more than 4 feet shall be guarded by one or more of the barriers specified in paragraph (b)(1) of this section or as required by the conditions.    a) On the first floor just north of the bailer, employee(s) were exposed to an 12 foot fall hazard due to an unprotected wall opening where workers remove refuse from the basement.

29 CFR 1910.147(d)(4)(i):     Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees.    (a) In the batching area, a worker was exposed to moving parts within the Hayes & Stolz volumetric coating system due to the equipment having been taken apart to change the sleeves with the power not locked out.    (b) In areas  throughout the facility, employees were exposed to moving parts due to the unclogging of grain or product from conveyor legs without the system having been locked out.

29 CFR 1910.219(c)(2)(i):     All exposed parts of horizontal shafting seven (7) feet or less from floor or working platform, excepting runways used exclusively for oiling, or running adjustments, shall be protected by a stationary casing enclosing shafting completely or by a trough enclosing sides and top or sides and bottom of shafting as location requires.    (a) In the basement, employees performing general cleaning and inspection were exposed to unguarded rotating shafts and couplings on the motor drives for the Hammer mill and the (2) Kelly horizontal mixers.
