29 CFR 1910.219(e)(3)(i): Vertical and inclined belts shall be enclosed by a guard conforming to standards in paragraphs (m) and (o) of this section.    a) Outside main motor drive belt for the auger was not effectively closed in that the supporting screws were missing allowing  a large opening in the cover exposing employees to pinch points.

29 CFR 1910.305(g)(2)(ii): Flexible cords may be used only in continuous lengths without splice or tap. Hard-service cord and junior hard-service cord No. 14 and larger may be repaired if spliced so that the splice retains the insulation, outer sheath properties, and usage characteristics of the cord being spliced.    a) Cord used in the loading area had damage to the outer insulation and the cord end had been repaired with electrical tape.

29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder. The railing shall be provided with a toeboard wherever, beneath the open sides,    a) Standard guardrailing system or equivalent protection was not provided on Bin #11 platform attached on the right side of the climbing ladder to prevent employees exposure to falls in excess of 9 feet.    b) Standard guardrailing system or equivalent protection was not provided on Bin #12 platform attached to the left side of the climbing ladder to prevent employees exposure to falls in excess of 10 feet.    c) Standard guardrailing system or equivalent protection was not provided on Bin #13 platform attached to the right side of the climbing ladder  to prevent employees expsure to falls in excess of 13 feet.    The Sabina Farmers Exchange was previously cited for a violation of this occupational safety and health standard or its equivalent standard  1910.23(c)(1), which was contained in OSHA inspection number 315040154, citation number (1), item number (2) and was affirmed as a final order on February 16, 2011, with respect to a workplace located at 292 N. Howard Street, Sabina, OH 45169.
