Portland cement Storage Warehouse- On or about December 01, 2011 and at prior times thereto, damaged bags of Portland cement were stored in the building and dry cement powder was laying on the floor throughout the warehouse.

29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer: (Construction Reference 1926.103) a) Conwood Products, Inc. Portland Cement Storage Warehouse- On or about December 01, 2011 and at times prior thereto, an employees wore a NIOSH approved 3M N95 filtering face piece respirator during stacking and delivering of Portland cement to customers. The employer did not provide the information in Appendix D of the Respiratory Protection Standard.
