29 CFR 1910.23(e)(2): Stair rails did not consist of a top rail, intermediate rail and/or posts, with the vertical height not more than 34 inches (86.36 cm) nor less than 30 inches (76.2 cm) from upper surface of top rail to surface of tread in line with face of riser at forward edge of tread: Warehouses A & B - The stair railings leading to the second floor mezzanines in both warehouses were inadequate in the following ways: a. Warehouse A - Both the left and right side stair railings were less than 30" in height. Measurements ranged from 26"-28". b. Warehouse B - The stair railing was less than 30" in height. Measurements ranged from 26"-28". *ABATEMENT NOTE - Warehouse A - The 11th stair of the right side stairs leading to the storage mezzanine was cracked completely through and needs to be replaced.

29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification which included the requirements as outlined in 29 CFR 1910.132(d)(2): Warehouses - Employees have been provided personal protective equipment such as gloves and safety shoes to protect them against hazards such as sharp edges or heavy materials falling on their feet in warehouses, face shields and disposable masks when weed whacking, and safety glasses when cutting metal. A hazard assessment has not been performed to include the following elements: 1. Select and have each affected employee use the types of PPE that will protect them from the hazards identified in the hazard assessment; 2. Communicate the selection decisions to each affected employee and; 3. Select PPE that properly fits each affected employee. *ABATEMENT NOTE: The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.
