29 CFR 1926.95(d)(1): The employer failed to provide the protective equipment, including personal protective equipment (PPE), used by employees to comply with OSHA regulations, at no cost to employees: a) Vaughn Industries, Inc On or about February 8, 2012, at the work site near 6353 State Route 366, Huntsville, OH: the employer did not provide protective equipment at no cost to employees who were exposed to cuts and lacerations from tasks such as, but not limited to, wire stripping. Employees were required to purchase leather work gloves to protect their hands from lacerations caused by daily work activities. Changed to: a) Vaughn Industries, Inc., on or about February 8, 2012, at the work site near 6355 State Route 366, Huntsville, OH: the employer did not assure that hazard assessments clearly identified that protective equipment was provided at no cost when the hazard assessment identified that cut and laceration hazards existed, such as, but not limited to, wire stripping. Instance B deleted b) Vaughn Industries, Inc On or about February 8, 2012, at the work site near 6353 State Route 366, Huntsville, OH: the employer did not provide protective equipment at no cost to employees who were exposed falls when performing climbing activity. Employees were required to purchase their own lineman belts and hooks to protect them from fall hazards when climbing utility poles to perform work.
