29 CFR 1910.119(e)(3)(i): The process hazard analysis did not address the hazards of the process: a) Gonnella Frozen Food LLC. - The employers process hazards analysis did not address hazards of its ammonia refrigeration process that were not controlled as prescribed by the ANSI/ASHRAE (American Society of Heating, Refrigerating and Air Conditioning Engineers) Standard 15 - Safety Standard for Refrigeration Systems and IIAR (International Institute of Ammonia Refrigeration) Bulletin No. 109 - Guidelines for IIAR Minimum Safety Criteria for a Safe Ammonia Refrigeration System, or an equivalent. Such hazards include, but are not limited to: 1) Emergency shutdown procedures including precautions to be observed in the event of a breakdown or leak, were not posted outside the engine room and immediately adjacent to each door. 2) Emergency ventilation fans were on a separate electrical circuit and have a control switch immediately outside the machinery room door.

29 CFR 1910.119(e)(5): The employer did not establish a system to assure that the process hazard analysis team's recommendations are resolved in a timely manner and that the resolution is documented: a) Gonnella Frozen Products LLC. - The employer did not establish a system to assure that the process hazard analysis team's recommendations for Management of Change 2011 PHA were resolved in a timely manner and that the resolution is documented, specifically: 1) Recommendation 1: Verify that the Recirculator vessel is rated for -40 F. 2) Recommendation 2: Consider adding soft hot gas to new evaporators. 3) Recommendation 3: Verify that there is a check valve on EV14 and EV15. 4) Recommendation 4: Verify that only one evaporator can manually be put into defrost. 5) Recommendation 5: Verify type of pipe stands be used. 6) Recommendation 6: Verify that the ammonia detectors are tied in to shut down the HVAC units on the roof. 7) Recommendation 7: Evaluate the wind sock location. 8) Recommendation 8: Relocate the wind sock to the west fence. 9) Recommendation 9: Consider adding an ammonia detector on the new recirculator relief header. 10) Recommendation 10: Provide light at the new recirculator on the roof. 11) Recommendation 11: Evaluate the stairs on the roof. 12) Recommendation 12: Consider adding pumpout to Freezer #1 at EV10 and EV12. 13) Recommendation 13: Consider adding tie-in from -25 Recirculated liquid line to 10 -40F suction line. 14) Recommendation 14: Consider Adding pumpout to Freezer # 2 at EV7.
