Main Bay - Observed on or about 1/23/12, a General #31 drill press, model 34-02, serial # AF2229 did not have a guard at the point of operation. b. 60 K Bay - Noted on or about 1/23/12, a 15" Wilton drill press (#34), serial #193706, was missing a guard at the point of operation. c. Bldg. 139, Vehicle Maintenance - Noted on or about1/20/12, the rotating chuck on a Monarch Machine Tool horizontal lathe, serial # 37339, was not provided with safeguarding to prevent unintentional contact.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: a. 60K Bay - Noted on or about 1/23/12, a Dayton 8" bench grinder had a measured 1/2" gap between the tongue guard and the grinding wheel. b. Main Bay - Noted on or about 1/23/12, a Delta bench grinder, MD. #23-850, serial #42-85, had a 1" measured gap between the tongue guard and the abrasive wheel.

Bldg. 549, Storage Area - Noted on or about 1/24/12, an electrical panel board, Panel B/120V/240V, had a hole in the circuit breaker which was observed covered with cardboard and tape. The whole measured 3"x 2" and energizes het and lighting.
