29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.    a.   Main Bay  - Observed on or about 1/23/12, a General #31 drill press, model 34-02, serial # AF2229 did not have a guard at the point of operation.    b.   60 K Bay - Noted on or about 1/23/12, a 15"  Wilton drill press (#34),  serial #193706, was missing a guard at the point of operation.    c.  Bldg. 139, Vehicle Maintenance - Noted on or about1/20/12, the rotating chuck on a Monarch Machine Tool horizontal lathe, serial # 37339, was not provided with safeguarding to prevent unintentional contact.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:    a.   60K Bay - Noted on or about 1/23/12, a Dayton 8" bench grinder had a measured 1/2" gap between the tongue guard and the grinding wheel.     b.  Main Bay - Noted on or about 1/23/12, a Delta bench grinder, MD. #23-850, serial #42-85, had a 1" measured gap between the tongue guard and the abrasive wheel.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:       a.  Bldg. 549, Storage Area - Noted on or about 1/24/12, an electrical panel board, Panel B/120V/240V, had a hole in the circuit breaker which was observed covered with cardboard and tape.  The whole measured 3"x 2" and energizes het and lighting.

Bldg. 139, Vehicle Maintenance, Machine Shop - Noted on or about 1/23/12, a flexible cord which energized (110V) lighting above the workbench was damaged through to the insulation.
