29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: Where shelving attachments were welded onto the forks of at least three stand-up Raymond OPC30TT forklifts, the compromised rated lifting capacity was not marked on the forklifts. These forklifts were stored and operated in the following locations: A. Bldg. 549 Warehouse B. Bldg. 861 Material Management Flight The U.S. Air Force 60th Logistics Readiness Squadron was previously cited for a violation of this occupational safety and health standard 1910.178(a)(5), which was contained in OSHA inspection number 189976, citation number 01, item number 003b and was affirmed as a final order on 08/22/2012, with respect to a workplace located at 350 Hangar Avenue Travis AFB, CA 94535.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure: Bldg. 554 Fuels: An anti-sparking extension cord was used in lieu of permanent wiring to energized computer equipment. The U.S. Air Force 60th Aerial Port Squadron was previously cited for a violation of this occupational safety and health standard 1910.305(g)(1)(iv)(A), which was contained in OSHA inspection number 190542, citation number 01, item number 004a and was affirmed as a final order on 08/22/2012, with respect to a workplace located at 90 Ragsdale Street Travis AFB, CA 94535.
