29 CFR 1910.134(c)(2)(ii):     The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user:    Employees working on the paint line voluntarily wore tight-fitting half-mask elastomeric respirators and the employer has not established and implemented all the all the elements required in a written program to ensure safe respirator use.

29 CFR 1910.134(e)(1):     The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:    Employees working on the paint line voluntarily wore tight-fitting half-mask elastomeric respirators and the employer did not provide medical evaluations prior to the use of respiratory protection in the workplace.

29 CFR 1910.134(h)(2)(i):     Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or  were not packed or stored to prevent deformation of the facepiece and exhalation valve:    Employees working on the paint line voluntarily wore tight-fitting half-mask elastomeric respirators and respirators were hung in the work area when not in use. The respirators were not properly stored to protect them from damage.

29 CFR 1910.134(k):     The employer did not provide comprehensive, understandable training which did not occur annually and/or more often if necessary:    Employees working on the paint line voluntarily wore tight-fitting half-mask elastomeric respirators and were not provided with comprehensive training to ensure that employees understand and demonstrate the following:          Why the respirator is necessary and how improper fit, usage, or maintenance can compromise the protective effect    of the respirator;        What the limitations and capabilities of the respirator are;        How to use the respirator effectively in emergency situations, including situations in which the respirator malfunctions;        How to inspect, put on and remove, use, and check the seals of the respirator;        What the procedures are for maintenance and storage of the respirator;        How to recognize medical signs and symptoms that may limit or prevent the effective use of respirators
