29 CFR 1926.1053(b)(1):     Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.    On or about February 29, 2012 employees were accessing a roof from a ladder that did not extend three feet above the landing exposing employees to a fall hazard.    Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c)

29 CFR 1926.451(g)(1):     Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.    On or about February 29, 2012 employees were working from a scaffold capping a chimney without fall protection exposing employees to a fall hazard of greater than 25 feet.    Woodridge Enterprises, Inc. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 1926.451 (g)(1) which was contained in OSHA inspection number 314031824, citation number 2, item number 1, and was affirmed as a final order on 02/17/2010, with respect to a workplace located at 362 Rex Blvd, Elmhurst, Il 60126.    Woodridge Enterprises, Inc. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 1926.451 (g)(1) which was contained in OSHA inspection number 312722721, citation number 1, item number 4a, and was affirmed as a final order on 03/10/2009, with respect to a workplace located at Garfield & Ninth Street, Hinsdale, IL 60521.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d)
