29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    a)  On January 29, 2018, employees engaged in hoisting operations exposed to struck-by hazards were not wearing hard hats.    Woodridge Enterprises, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a) which was contained in OSHA inspection number 1194723, citation 1, item 1, and was affirmed final order on March 2, 2017, with respect to a workplace located at 1613 Cambridge Dr., Flossmoor, IL 60422.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):       a)  On January 29, 2016, employees performing roofing work were exposed to fall hazards of approximately 20.5 feet.    Woodridge Enterprises, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 958532, citation 1, item 1, and was affirmed final order on April 29, 2014, with respect to a workplace located at 4900 Grand Ave., Western Springs, IL 60558.      Woodridge Enterprises, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1002987, citation 1, item 1, and was affirmed final order on May 1, 2015, with respect to a workplace located at 515 Maple, Hinsdale, IL 60521.      Woodridge Enterprises, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1088008, citation 1, item 1, and was affirmed final order on October 21, 2015, with respect to a workplace located at 13616 S. Potawatomi Trail, Homer Glen, IL 60491.      Woodridge Enterprises, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1194723, citation 2, item 1, and was affirmed final order on March 2, 2017, with respect to a workplace located at 1613 Cambridge Dr., Flossmoor, IL 60422.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
