On or about December 2, 2016, at the above addressed jobsite, an employee working on the ground, below roofing activities, was not wearing a hard hat to protect against falling materials and debris.

On or about December 2, 2016, at the above addressed jobsite, employees were exposed to slip, trip and fall hazards when the bottom of the ladder was not kept clear of debris, including but not limited to, piles of discarded shingles and nails.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    a.  On or about December 2, 2016, at the above addressed jobsite, employees performing residential roofing operations on a roof approximately eight (8) feet eight (8) inches above the ground, were not protected from falls, thereby exposing the employee to fall hazards.      Woodridge Enterprises, Inc. was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard 1926.501 (b)(13) which was contained in OSHA inspection number 1088008, citation number 1, item number 1 and was affirmed as a final order on April 7, 2016 with respect to a workplace located at 13616 South Potawatomi Trail, Homer Glen, IL, 60491.    Woodridge Enterprises, Inc. was previously cited for a willful violation of this occupational safety and health standard or its equivalent standard 1926.501 (b)(13) which was contained in OSHA inspection number 1002987, citation number 1, item number 1 and has not been affirmed with respect to a workplace located at 515 Maple, Hinsdale, IL, 60521.    Woodridge Enterprises, Inc. was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard 1926.501 (b)(13) which was contained in OSHA inspection number 958532, citation number 1, item number 1 and was affirmed as a final order on May 13, 2015 with respect to a workplace located at 4900 Grand Avenue,  Western Springs, IL, 60558.    Woodridge Enterprises, Inc. was previously cited for a serious violation of this occupational safety and health standard or its equivalent standard 1926.501 (b)(10) which was contained in OSHA inspection number 279570, citation number 1, item number 1 and was affirmed as a final order on September 27, 2013 with respect to a workplace located at 1175 N. Gary Avenue, Carol Stream IL 60188.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:    a.  On or about December 2, 2016, at the above addressed jobsite, an employee conducting roofing operations from a ladder jack scaffold, approximately seven (7) feet  above the ground, was not provided with an access ladder, thereby exposing the employee to falls.      Woodridge Enterprises was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451 (e)(1) which was contained in OSHA inspection number 829161, citation  number 1, item number 2 and was affirmed as a final order on August 21, 2013 with respect to a workplace located at 634 Hickory, Hinsdale, IL 60188
