29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a. On or about November 16, 2021, at the above addressed jobsite, an employee using pneumatic nail guns was not wearing safety glasses, thereby exposing employee to hazards associated with eye injuries.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:  a.  On or about November 16, 2021, at the above addressed jobsite, employees performing residential roofing operations on a roof, eleven (11) feet six (6) inches above the ground, were exposed to fall hazards due to the lack of fall protection.  Woodridge Enterprises, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1448875, citation number 1, item number 1 and was affirmed as a final order on April 8, 2020, with respect to a workplace located at 825 Merganser Lane, Peotone, IL 60468.  Woodridge Enterprises, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1291048, citation number 1, item number 2 and was affirmed as a final order on May 23, 2018, with respect to a workplace located at 6531 Longmeadow Ave., Lincolnwood, IL 60712.  Woodridge Enterprises, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501 (B)(13), which was contained in OSHA inspection number 1194723, citation number 2, item number 1 and was affirmed as a final order on March 2, 2017, with respect to a workplace located at 1613 Cambridge Dr., Flossmoor, IL 60422.

On or about November 16, 2021, at the above addressed jobsite, employees had been provided fall protection training, however, the employer failed to keep a written certification of record.
