29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:      On March 6, 2012, and at times prior thereto, in the following locations, openings in circuit breaker panels were not covered by blanks;      A. San Onofre Exchange by Customer Service Area: Panel L1A which energized lighting throughout the building had several openings inside the cabinet due to an unsecured faceplate and uncovered spare spaces which exposed the live inner conductors.     B. San Onofre Exchange by stairs to upper mezzanine: Panel P2A which energized ventilation, lighting, and a freezer had several uncovered spare spaces which exposed the live inner conductors.    C. Retail Warehouse: The Intermatic Dock Light Timer was missing an internal guard and the live inner conductors were exposed.    D. Country Store: A Siemens Panel had an uncovered spare space and live inner conductors were exposed.
