29 CFR 1910.178(a)(5): Where powered industrial trucks were equipped with front-end attachments other than factory installed attachments, the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elvation with the load laterally centered: Warehouse #12108: On March 6, 2012, and at times prior thereto, where five foot eight inch extensions for the Mitsubishi forklift was used, the forklift did not have a name plate to indicate the truck's lifting capacity.
