29 CFR 1910.132(d)(2): The employer did not verify, through a written certification, that the required workplace hazard assessment had been performed:    (a) Xtreme Drilling and Coil Services, Inc. at Howard 24N-29- HZ in Wattenburg, CO: The employer did not verify, through written certification, that the required workplace hazard assessment had been performed.  Personal protective equipment provided for activities at Rig 20 was not documented in a workplace hazard assessment.  This condition exposed employees to chemical and physical hazards.    Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1910.1200(e)(1)(i): The written hazard communication program did not include a list of the hazardous chemicals known to be present, using an identity that was referenced on the appropriate material safety data sheet:    (a) Xtreme Drilling and Coil Services, Inc. at Howard 24N-29 HZ in Wattenburg, CO: The written hazard communication program did not include a list of the hazardous chemicals known to be present on Rig 20.  Some of the chemicals observed onsite were Barite, Quick Slide, 123L, and Aspersol SUP.This condition exposed employees to a chemical hazard.    Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1910.1200(e)(2): When the employer produced, used, or stored hazardous chemicals at a workplace in such a way that the employees of other employer(s) may be exposed (for example, employees of a construction contractor working on-site) the employer did not ensure that the hazard communication addressed paragraphs (i) through (iii) of this section:    (a) Xtreme Drilling and Coil Services, Inc. at Howard 24N-29 HZ in Wattenburg, CO: The written hazard communication program did not include the methods the employer will use to inform contractors of the chemicals produced, used, and stored at Rig 20 and the pertinent hazards of those chemicals.  This condition exposed employees to a chemical hazard.    Abatement Note: For multi-employer worksites the HAZCOM program must specify:  The methods the employer will use to provide the other employer(s) on-site access to safety data sheets for each hazardous chemical the other employer(s)' employees may be exposed to while working;  The methods the employer will use to inform the other employer(s) of any precautionary measures that need to be taken to protect employees during the workplace's normal operating conditions and in foreseeable emergencies; and,  The methods the employer will use to inform the other employer(s) of the labeling system used in the workplace.
