29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazards created by rotating parts: (a) Xtreme Drilling and Coil Services, Inc., Rig #6 at Ratter 4C-34HZ in Platteville, CO 80651: On 5/30/12, the employer did not protect employees from hazards created by damaged factory guard adjacent to pre-charge pump. This condition exposed employees to an amputation hazard. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
