29 CFR 1910.23(c)(1): Open sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides:    (a)	Xtreme Drilling and Coil Services, Inc., Rig #20 at Howard 24N-29 HZ in Wattenburg, CO 80621: On 3/21/12, the employer did not ensure that the railing on the rig floor and mud pit were equipped with a standard railings to guard against falling in that there was no guardrails. 29 CFR 1910.23(d)(1): Flights of stairs having 4 or more risers were not equipped with standard stair railings or standard handrails on each side:    (a)	Xtreme Drilling and Coil Services, Inc., Rig #20 at Howard 24N-29 HZ in Wattenburg, CO 80621: On 3/21/12, the employer did not ensure that flights of stairs with 4 or more risers were equipped with a standard handrail on each side. A stair with 4 risers or more provided access to the pipe rack. The handrail was missing. This condition exposed employees to a fall hazard.      Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    (a)	Xtreme Drilling and Coil Services, Inc., Rig #20 at Howard 24N-29 HZ in Wattenburg, CO 80621: On 3/21/12, the eyewashes for quick drenching or flushing of the eye in the hopper house and on the mud pit were not functional in that the bladder had no water and not properly installed.  This condition exposed employees using corrosive chemicals to an eye hazard.                a)	Hopper House              b)	Mud Pit    Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
