OSH ACT of 1970 Section (5)(a)(1):  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to amputation hazards:      a)  Xtreme Drilling and Coil Services, Inc. at Rig #14 at Hidden Creek #36-29, Frederick, CO 80516:  On or about April 5, 2012, the employer did not ensure that the drawworks was locked out when replacing the Sala block near the crown.  Condition exposed employee to amputation hazards.    Among other methods, one feasible and acceptable abatement method is to lock out and tag out the drawworkds in accordance with American National Standards Institute standard Z244.1-2003 Section 5.3.2.5, which provides, "Lockout or tagout devices shall be affixed to each energy isolation device by authorized individuals."    In addition, American Petroleum Institute (API) Recommended Practice for Occupational Safety for Oil and Gas Well Drilling and Servicing Operations #54 Section 6.9 provides, "Locks and/or tags should be placed to plainly identify the equipment or circuits being worked on."  And, "Personnel should be trained or disciplined in the use of this system to prevent unexpected operation of any equipment that stores any type of energy that might inflict injury to personnel."

OSH ACT of 1970 Section (5)(a)(1):  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to caught in-between and fall hazards:      a)  Xtreme Drilling and Coil Services, Inc. at Rig #14 at Hidden Creek #36-29, Frederick, CO 80516:  On or about April 5, 2012, the employer provided the Rotzler Titan TH4 hydraulic winch for use in lifting personnel when this lift was not designed or rated as a personnel hoist.  This winch did not have a hoist limiting devices such as an anti two-block system, and was rated to lift up to 176 ft/min.  In addition, the hoist was not designed with a braking sytem to prevent employee injury with sudden braking.  Condition exposed employee to caught in-between and fall hazards.    Among other methods, one feasible and acceptable abatement method is to use a hoist that is designed and rated as a manlift, and which is explosion proof, such as a pneumatic hoist.
