29 CFR 1926.451(e)(8): Direct access to or from another surface shall be used only when the scaffold is not more than 14 inches (36 cm) horizontally and not more than 24 inches (61 cm) vertically from the other surface. At the site, employees were stepping from one section of tubular welded frame scaffolding to gain access to the perpendicular section of scaffolding. The distance between the two sections of scaffolding was at 25-1/2" at the bottom planks on the top of the first buck and at a greater distance farther up the scaffold onto the second buck, thereby exposing the employees to a fall hazard at a max height of 12 feet.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. At the site, an employee was performing masonry work three bucks high on the top set of planking without a guardrail system installed or any other means to protect the employee from a fall of 18 feet to the next lower surface.

29 CFR 1926.451(g)(4)(i): Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews. At the site, there were employees performing masonry work on a fabricated frame scaffold without a guardrail system installed on the ends of the platform, thereby exposing the employees to a fall hazard from a height of 12 feet to the next lower surface.
