29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access.    a. On the west side of the building the employer failed to provide employees a means of egress and employees were required to climb the crossbraces; thereby exposing employees to a fall hazard.      Vibo Construction was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 461813, citation number 1, item number 4 and was affirmed as a final order on January 14, 2013, with respect to a workplace located at 5215 Avery Road, Dublin, OH 43016.

Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews.    a. The on the northwest corner of the building employer failed to install a guard rail on the end portion of the scaffold, thereby exposing employees to a fall hazard of 14 feet. 29 CFR 1926.451(h)(1): In addition to wearing hardhats each employee on a scaffold shall be provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, or guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects. When the falling objects are too large, heavy or massive to be contained or deflected by any of the above-listed measures, the employer shall place such potential falling objects away from the edge of the surface from which they could fall and shall secure those materials as necessary to prevent their falling.    a. The employer was storing pallets of brick and mortar on the scaffold and side brackets without providing employees with a means of deflection or protection from falling on employees below. The employees were exposed to a struck by hazard.
