29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On the manufacturing floor the bending brake #2 was provided with a single hand control mounted to the face of the machine for operation of the clamping device. The operation placed the operator in a position to accidentally place his hand into the point of operation and potentially suffer crushing or amputation injuries. Abatement certification and abatement document is required for this violation. The documentation should include written verification of abatement, applicable measurement or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citation(s).

29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: (a) As part of the documented energy control (Lock Out Tag Out) program, written procedures for each energy control device were deficient for but not limited to the #96 Loom . The stop button on the main control panel for the #96 loom was indicated as the single disconnect requiring lockout. Visual observation of a lockout demonstration indicated the system also required shut off and lock out of the supplied air valve. In addition testing and relief of potential stored energy in the hydraulic system was not indicated. Abatement verification is required for this violation. The abatement certification sheet is enclosed with the citation(s).
