29 CFR 1926.25(a):   During the course of construction, alteration, or repairs,  debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:  On or about February 23, 2012, employees were cutting materials and performing other construction work inside the building under construction with cords, raw and cut lumber and other debris scattered throughout the work area, exposing employees to the hazard of slips, trips and falls.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that materials, equipment and trash are organized in a manner that keeps work areas and passageways clear.

29 CFR 1926.102(a)(1):   Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  On or about February 23, 2012, employees were utilizing pneumatic nail guns to frame the building under construction without the employer providing eye protection, exposing the employees to the hazard of being struck by flying objects.   Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that each employee is provided eye and face protective equipment while operating equipment such as, but not limited to pneumatic nail guns.

29 CFR 1926.404(f)(6):   The path to ground from circuits, equipment, or enclosures was not permanent and continuous:  On or about February 23, 2012, employees were utilizing a 120V yellow extension cord that was not equipped with a ground prong to power a circular saw, exposing the employees to an electrical shock hazard.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all cords requiring a ground prong are equipped with one when in use on construction sites.

Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all flexible cords are provided strain relief.

29 CFR 1926.416(e)(1):   Worn or frayed electric cords or cables were used:  On or about February 23, 2012, employees were utilizing a circular saw that was powered with a yellow 120V extension cord where the sheathing was cut and frayed, which exposed the insulated wires, exposing the employees to an electrical shock hazard.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that the sheathing on all flexible cords is not worn or frayed.

29 CFR 1926.501(b)(1):   Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that each employee working over 6 feet high is provided fall protection.  The Puckett Construction Company was previously cited for a violation of this Occupational Safety and  Standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in Inspection  Number: 314279738, Citation Number: 01, Item: 01, issued on: July 20, 2011, and was affirmed as a Final Order on August 19, 2011, with respect to a workplace located at: 101 Industrial Blvd., Euless, TX 76039.
