29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section. On the west side of the construction site, an employee installing Tyvek material from a basket elevated by a Genie GTH-844 forklift without fall protection was exposed to a fall hazard of approximately 12 feet to the ground below.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift. On the west side of the construction site, employees elevated by a Genie lift without fall protection to install Tyvek material. The employees were exposed to fall hazards of approximately 12 feet to the ground level.
