29 CFR 1926.502(d)(16)(iii):     Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:    a) On or about 4/4/12 at the Wyoming County Community Hospital located at 400 North Main Street, Warsaw, New York; employees preparing a roof deck for a replacement roof using harnesses and rope grabs for work at the edge of the roof. The anchorage point for the rope was approximately 12 feet from the edge of the roof and the employee's rope grab was adjusted to approximately 28 feet allowing for a free fall of approximately 16 feet.     NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.503(a)(2):     The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii)    a) On or about 4/4/12 at the Wyoming County Community Hospital located at 400 North Main Street, Warsaw, New York; employees were not trained in the proper use and rigging of the lifeline and rope grab system. The rope grab was positioned on the lifeline at a point that would have allowed the employee to free fall farther than 6 feet.    b) On or about 4/4/12 at the Wyoming County Community Hospital located at 400 North Main Street, Warsaw, New York; employees were not trained in the proper use of personal fall arrest equipment. An employee wearing a harness had the lanyard attached backwards on the harness.    ABATEMENT CERTIFICATION REQUIRED
