29 CFR 1910.147(c)(4)(i): Procedures were developed and documented but were not utilized for the control of potentially hazardous energy. (a) On, or before, April 4, 2012, operators were exposed to a caught in hazard by entering a caged area of a press brake to perform machine service. Employees relied on a form of machine guarding; a door mounted magnetic shut-off switch to interrupt electrical power, instead of using a lock-out procedure used by maintenance personnel to isolate hazardous energy. Abatement Note: Abatement certification is not required for this item.
