Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falls, struck-by, and/or crushing hazards:    (a)  On or about April 2, 2012, for the employees unloading lengths of polyethylene pipe from a flatbed semi-trailer by pushing each pipe off the back of the trailer as the truck was driving through a pasture. The truck was climbing up an incline when it began to bounce causing the pipe to slide off the trailer.  When an employee tried to hold down the pipe, he was flung off of the trailer and struck his head against the ground.    Abatement Note:  Among other methods, one feasible and acceptable method to correct this hazard would be to follow the Plastics Pipe Institute - Material Handling Guide.
