29 CFR 1926.453(b)(2)(v): A safety harness was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift. a) Job site, 200 Adams Ave. Mineral Point, PA: On or about May 22, 2023, two employees were not wearing any fall protection while they were working from an aerial lift. The employees were doing roofing activities. The fall hazard to the two employees was approximately up to 30 feet above a concrete parking lot.

29 CFR 1926.454 (a)(3): The employer did not train each employee who performs work on a scaffold, by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards, including the proper use of the scaffold and the proper handling of materials on the scaffold. a) Job site, 200 Adams Ave. Mineral Point, PA: On or about May 22, 2023, the employer did not train the employees to safely work out of an aerial lift basket. The employees were not wearing any fall protection while they were working approximately up to 30 feet above the concrete parking lot.
