(a) On or about April 16, 2013 at 1815 NW 102 TER Kansas City Missouri, 64157. Employees were exposed to a fall hazard while working at heights of approximately 13-15 feet without proper fall protection.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (a) On or about April 16, 2013 at at 1815 NW 102 TER Kansas City Missouri, 64157. Employees was exposed to a fall hazard by using a ladder which did not extend 3 feet above the landing surface of the roof to which he gained access.
