29 CFR 1910.22(b)(1): Aisles and passageways were not kept clear, with no obstruction across or in aisles that could create a slip or trip hazard. Rig #11: On or about April 11, 2012, walking/working platform metal suitcases, which facilitate most of the rigs electrical wiring were damaged at two locations: a) Location #1; two metal suitcases to the left from where the L-shape begins, the lid on from the metal suitcase would not close properly; it lay open 3 inches above the rest of the platform. b) Location #2; two metal suitcases down from where the L-shape begins, the lid on another metal suitcase was bent inward, it lay dented inward 1 to 1 1/2 inches below the rest of the platform.

29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk was not guarded: Rig #11: On or about April 11, 2012, where the red pop-off line went into the mud pit walking/working platform, a floor hole was left unprotected. The floor hole opening measured 13 1/2 inches by 12 1/2 inches. Employees were exposed to fall hazards.

29 CFR 1910.24(f): Rise height and tread width were not uniform throughout any flight of stairs including any foundation structure used as one or more treads of the stairs: Rig #11: On or about April 11, 2012, four instances where; stairways were not uniform & level and/or broken. a) Left side stairway, leading up to the mud pit; second and top stair broken. b) Right side stairway, leading up to the mud pit; the third through seventh stair bent. c) Front stairway leading up to the rig floor; second and third stair broken. d) Back stairway leading up to the shaker area; first, second, third and fourth stair bent.
