29 CFR 1910.106(b)(6): In locations where flammable vapors may be present, precautions were not taken to prevent ignition by eliminating or controlling sources of ignition. Sources of ignition may include open flames, lightning, smoking, cutting and welding, hot surfaces, frictional heat, sparks (static, electrical, and mechanical), spontaneous ignition, chemical and physical-chemical reactions, and radiant heat: On or about 05/04/12, and at times prior thereto, employees were exposed to an electrical/explosion hazard while working near the flammable diesel tanks storage area which had a loose bonding/grounding connector that connected the tanks to the earth ground rod.

29 CFR 1910.303(c)(3): Conductors were not spliced or joined with splicing devices suitable for the use, or by brazing, welding, or soldering with a fusible metal or alloy: On or about 05/04/12, and at times prior thereto, employees were exposed to an electrical/explosion hazard while working near the SCR House which was being used to store flammable paint. Inside the SCR House was an operating spark producing electrical fan which had a spliced extension cord. This splice extension cord was wrapped with black electrical tape.
