On or about April 11, 2012, Rig #62, employees were exposed to chemical hazards in that the employer had not installed an eye wash station for employees in the chemical dispensing area.

Safety of equipment was not determined using the following considerations: On or about April 11, 2012, Rig # 62, employees were exposed to electrical hazards in that the employer did not ensure that the outer insulation on a flexible cord for the light was not damaged.

NOMAC DRILLING, LLC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1910.305(g)(5), WHICH W2AS CONTAINED IN OSHA INSPECTION NUMBER 311685267, CITATION NUMBER 1, ITEM NUMBER 2, ISSUED ON MARCH 16, 2009, WITH RESPECT TO A WORKPLACE LOCATED AT LUCKY D 4H, LITTLETON, WV.
