29 CFR 1910.269(c)(1):     Additional job briefings were not held when a significant job change affected the safety of the employees during the course of the work:      a)  Rt. 88 & Arnold Avenue, Pt. Pleasant, NJ:  The 3 man crew on site did not conduct an additional safety meeting when the employees were preparing to attach the sling to the pot and realized that the area to attach was not in a safe reaching distance to the placement of the bucket truck, occured on or about 4/17/12.    NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.67(c)(2)(iv):     Employees did not always stand firmly on the floor of the basket, and were allowed to sit or climb on the edge of the basket or use planks, ladders, or other devices for a work position:    a)  Rt. 88 & Arnold Avenue, Pt. Pleasant, NJ:  The employee was not in a proper positon to reach the clamp on the top of the transformer for removal and reached around the steet side of the pole in order to put the lifting sling on the transformer which put him in close proximity to the energized transformer mounted arrester top.  The employee did not stand firmly on the floor of bucket truck #12412.  The employee wedged his feet up the inside of the bucket inorder to reach the transformer, observed on or about 4/17/12.      NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.269(l)(2)(ii):     The employer did not ensure that no employee(s) approached or took conductive objects closer to exposed energized parts than set forth in Tables R-6 through R-10 when the energized parts were not insulated from the employee and from any other conductive object of a different potential:    a)  Rt.88 & Arnold Avenue, Pt. Pleasant, NJ:  The employer did not ensure that minimum approach distances were maintained when an employee attempted to attach a lifting sling to the lifting ear of the middle transformer and made contact with the energized lightning arrester of the road side transformer.  The energized lightning arrester was not insulated from the employee, occurred on or about 4/17/12.    NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
