29 CFR 1910.269(a)(2)(ii)(D): The proper use of the special precautionary techniques, personal protective equipment, insulating and shielding materials, and insulated tools for working on or near exposed energized parts of electric equipment, where the employer failed to endure non-insulated tools were not utilized. A) Jersey Central Power & Light Company - On March 24, 2026, an employee sustained second?degree burns to the face and neck when a non?insulated tool contacted an exposed energized part, resulting in an arc flash. The employer failed to ensure that employees used insulated tools while securing the cut?out box, exposing them to electrical arc?flash and burn hazards in violation of 29 CFR 1910.269(a)(2)(ii)(D). Incident occurred on March 24, 2026

29 CFR 1910.269(l)(8)(v)(D): The employer did not ensure that each employee exposed to hazards from electric arcs wore protective equipment consisting of at a minimum a face shield with an arc rating greater than or equal to the heat energy estimated under paragraph (l)(8)(ii) of this section whenever that estimate exceeded 2.0 Cal/cm2 but less than 9.0 Cal/cm2. A) Jersey Central Power & Light Company - On March 24, 2026, an employee sustained second?degree burns to the face and neck when a non?insulated tool contacted an energized part, causing an arc flash. The employer failed to ensure that the employee, while working within the arc?flash boundary, wore the required arc?rated face protection (such as a face shield with an arc rating consistent with the estimated heat?energy exposure), as mandated by 29 CFR 1910.269(l)(8)(v)(D). Incident occurred on March 24, 2026
