29 CFR 1910.269(l)(4): The employer did not ensure that each employee, to the extent that other safety-related conditions at the worksite permitted, worked in a position from which a slip or shock would not bring the employee's body into contact with exposed, insulated parts energized at a potential different from the employee. (a) JCP&L Smithburg-Englishtown Right-of- Way located on the far west side of the Happy Day Farm located at 97 Daum Road: On August 3, 2012 an employee walking from his work truck to the area in which the crimping was being performed slipped/tripped/fell into a downed line resulting in his death, occurred on or about 8/3/12. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

An employee was fatally injured when he contacted a downed 34.5 kV line, occurred on or about 8/3/12. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
