29 CFR 1910.269(a)(3)(i)(A): Contract employer(s) were not informed by the host employer of the characteristics of the host employer's installation that are related to the safety of the work to be performed, as listed in paragraphs (a)(4)(i) through (a)(4)(v) of this section; a) Location-320 West Main St., Sayreville, NJ 08872- JCP&L Substation On or about 5/24/2024, contractor employee(s) conducted air quality testing within a JCP+L sub-station using hand-held multi-gas meter(s) and air sampling pumps, such as but not limited to, a Lighthouse 3016-iaq Particulate meter. Electrical lines and/or conductors housed within the sub-station were energized during air quality testing work by employee(s). The contract employer was not informed of the energized state and voltage(s) of lines and equipment housed in the building in which contractor work was to be performed, exposing contractor employee(s) to electrical hazard(s). An arc flash incident occurred on 5/24/2024, in which employee(s) sustained burns to both hands and face.

29 CFR 1910.269(a)(4): Existing characteristics and conditions of electric lines and equipment that were related to the safety of the work to be performed was not determined before work on or near the lines or equipment was started. Such characteristics and conditions include, but not limited to 1910.269(a)(4)(i) through 1910.269(a)(4)(viii): a) Location-320 West Main St., Sayreville, NJ 08872- JCP&L Substation On or about 5/24/2024, contractor employee(s) conducted air quality testing within a JCP+L sub-station using hand-held multi-gas meter(s) and air sampling pumps, such as but not limited to, a Lighthouse 3016-iaq Particulate meter. 29 CFR 1910.269(m)(3)(i): The employee that the employer designates pursuant to paragraph (m)(2) of this section as being in charge of the clearance did not make a request of the system operator to deenergize the particular section of line or equipment, as the designated employee in charge (as this term is used in paragraph (m)(3) of this section) and responsible for the clearance: a) Location-320 West Main St., Sayreville, NJ -JCP&L Substation, On or about 5/24/2024, a TRC Environmental Corporation contracted employee sustained electrical burns due to an arc flash while performing air quality testing within a JCP&L electrical power sub-station. A JCP&L supervisor and qualified employee designated to escort the TRC Environmental Corporation employee into the substation did not verify the building was de-energized prior to the start of work.
