29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: a) Lariat Drilling Rig #39: On or about April 12, 2012, and at times prior there to, employees mixing caustic chemicals were exposed to eye and skin chemical burns where an eye wash and shower were not immediatly available.

29 CFR 1910.305(g)(2)(ii): Flexible cords were not used in continuous lengths without splice or tap: a) Lariat Drilling Rig #39: On or about April 12, 2012, and at times prior there to, employees were exposed to shocks and burns from an electrical splice that was made on a flexible cord from a battery charger in the mud house.
