29 CFR 1910.215(a)(4): On offhand grinding machines, work rests were not used to support the work:  (a) In the maintenance shop, on or about April 23, 2012, and at times prior thereto, the Dayton pedestal grinder, model number 42123 H, work rest was not adjusted to within 1/8 inch of the 6 inch diameter composite wheel.  29 CFR 1910.215(a)(4): On offhand grinding machines, work rests were not used to support the work:  (a) In the maintenance shop, on or about April 23, 2012, and at times prior thereto, the Dayton pedestal grinder, model number 42123 H, work rest was not adjusted to within 1/8 inch of the 6 inch diameter composite wheel.  The work rest was positioned under housing used to enclose the abrasive wheel.

29 CFR 1910.215(b)(9): Exposure adjustment. Safety guards of the types described in Subparagraphs (3) and (4) of this paragraph, where the operator stands in front of the opening, was not constructed so that the peripheral protecting member can be adjusted to the constantly decreasing diameter of the wheel.   (a) In the maintenance shop, on or about April 23, 2012, and at times prior thereto, the Dayton pedestal grinder, model number 42123 H, did not have a tongue guard installed for the 6 inch diameter composite wheel.
